Dermal Fillers — doctor-administered at the hospital
Dermal fillers are gels — hyaluronic acid, a substance the skin already contains — placed under the skin to restore volume. As we age, fat pads shrink and descend and bone recedes; the result is hollowing under the eyes, flattening of the cheeks, and folds deepening from nose to mouth.
The skill is in restraint and in placement. Overfilled faces look overfilled precisely because volume was added where it was not lost. The aim here is that people notice you look well, not that you have had something done.
Hyaluronic acid fillers are reversible, which is a meaningful safety feature: if a result is not right, or in the rare event of a vascular complication, the filler can be dissolved with an enzyme. That reversibility is one reason this practice uses them rather than permanent alternatives.

Assessment
Where volume has actually been lost, which is often not where you think the problem is. Under-eye shadowing, for instance, is frequently pigment or skin laxity rather than hollowing — and filler would not help.
Numbing

Will it look obvious?
Not when it is done conservatively and placed where volume has actually been lost. Obvious filler is over-filled filler. Staging treatment across visits and reviewing at two weeks is how that is avoided.
Can it be reversed?
Yes — that is a key advantage of hyaluronic acid fillers. An enzyme called hyaluronidase dissolves them, whether because you are unhappy with the result or as an emergency measure. It is kept on site.
How long does it last?
Typically nine to eighteen months, varying by area and product. Lips tend to metabolise faster than cheeks because they move constantly.
Is filler the answer for my dark circles?
Only if they are caused by hollowing. Dark circles from pigmentation, visible blood vessels or thin skin will not improve with filler, and filling a hollow that is not there makes them look worse. That assessment is the first part of the appointment.
