Pigmentation & Melasma Treatment — doctor-administered at the hospital
Pigmentation is the most common reason people in India walk into a dermatology hospital, and the area with the most marketing noise around it. The useful starting point is that 'dark patches' is a description, not a diagnosis.
Melasma is symmetrical brown patching across the cheeks, forehead and upper lip, driven by hormones and sunlight, and it is a chronic relapsing condition — controlled well, but requiring maintenance. Post-inflammatory pigmentation is the mark left after acne or an injury, and it fades reliably. Tanning is sun-induced and reverses. Under-eye darkness is often structure and shadow rather than pigment at all, and no cream will change that.
Treatment is medical: prescribed topicals, sun protection that is actually used, and where suitable, peels or laser at settings appropriate for Indian skin. Aggressive treatment on pigment-prone skin makes it worse, which is why this sits on the medical side of the practice rather than the aesthetic one.

Identifying which type of pigmentation you have and how deep it sits — surface pigment responds far better and faster than deep pigment, and that changes what is realistic.
The non-negotiable foundation. Broad-spectrum sunscreen every morning, reapplied — without it, every other treatment is undone as fast as it works. This is the step patients skip and then wonder why nothing improved.
A regimen matched to your pigmentation type and your skin's tolerance. Not the same for melasma as for post-acne marks.
Chemical peels or laser, at conservative settings for Indian skin, once the skin is prepared and stable. Used to accelerate a working plan, never as the starting point.
Melasma relapses, particularly in summer and with hormonal shifts. A maintenance routine is part of the treatment, not an afterthought.

It is controlled rather than cured. Melasma is a chronic condition that relapses with sun exposure and hormonal change. With treatment and consistent sun protection it can be kept faint enough not to bother you — but stopping everything usually brings it back.
Eight to twelve weeks for a visible change in most cases; longer for deep or long-standing pigment. Anything promising a change in a fortnight is either superficial or unsafe.
Sometimes, and sometimes it makes it worse. Melasma in particular can flare after aggressive laser treatment on Indian skin. When laser is used here it is at conservative settings, after the skin has been prepared, and as part of a wider plan.
